Mitten Care Management is a nurse-led case management company based in Troy, Michigan. We provide expert care coordination for individuals recovering from auto injuries, workplace injuries, traumatic brain injuries, spinal cord injuries, chronic conditions, and through telehealth services.
We offer six core services: Auto Injury Care, Workers' Compensation Case Management, Chronic Care Support, Traumatic Brain Injury Recovery, Spinal Cord Injury Case Management, and Telehealth Case Management.
Our office is at 631 E Big Beaver Rd., Suite 211, Troy, MI 48083. We serve clients across Michigan and beyond, with telehealth options available.
Monday through Friday, 9:00 AM to 5:00 PM. For urgent cases, call anytime and leave a voicemail — we return calls within 24 business hours.
